Student Registration Form

Each student has to first fill out this form and be accepted by the Institute to begin the course of study.

Core Subject Registration & Payment Form

Before the start of each Core Subject, the student is required to fill in this form and mail it to the Institute Administrator with the relevant payment.

Please note all payments have to be made in advance and 3 weeks before the start of the term.

Seminar Registration & Payment form

Before attending a Seminar the student is required to fill in this form and mail it to the Institute Administrator with the relevant payment. One form can be used to list all those attending from one church.

Please note all payments have to be made in advance and 2 weeks before the start of the Seminar.
